Password or Username (lost password or change current password, note that your username can also be your email address:
- Go to online scheduling log in page.
- Click the “Forgot Password” link.
- Enter your email address used to register your account.
- Click “Next”.
- Check your email address for password reset email. This email is sent immediately. If you do not receive this email, check your spam folder.
- If you are not successful after repeated attempts, please complete an Appointment Request for this appointment, and we will call you. We can reset your password in person when you come in for your appointment.
